protected and medical leaves
requesting protected leave is a three step process:
Step 1: Notify 人力资源
You must provide your employer with a notice of intent to use protected leave benefits at least 30 days in advance of your need for leave (as required by the Oregon Employment Department).

In the instance of an emergency/unforeseen circumstance, you need to contact HR within 24 hours of the event and submit the above form within 3 days of starting the leave.
Step 2: Complete required leave documentation
人力资源 will reach out to you within 5 days of submitting your notification (step 1) and provide required paperwork:
Medical Leave Request (complete and return as soon as possible).
Certification of Health Care Provider (complete the employee portion and give to your medical professional).
Step 3: Return Medical Certification
It is your responsibility to make sure that the Certification of Health Care Provider document is returned to HR.
Some medical providers will fax the form directly to HR.
Kaiser will put the form into your employee portal; you will need to download and share with HR.
可选步骤4: Initiate a Paid Leave Oregon claim with The Standard
Accessing paid leave through The Standard is an optional benefit for eligible employees.
Call The Standard’s services center: 866-756-8115 (Mon-Fri, 5am-5pm).

Complete the paperwork that is sent from The Standard and return it directly to them.

You can take up to 12 weeks (60 days) in a 1-year period.

-You can take protected medical leave as Continuous or Intermittent.
-For 休假/OFLA, intermittent leave can be taken in full days or partial days.

-These are not paid leaves but we can use your accrued leave to keep you in a paid status.

Your job and benefits are protected while you are on an approved 休假/OFLA leave.
